Beginning with 10% of blue-collar workers in 1885, mandatory insurance has expanded; in 2009, insurance was made mandatory on all citizens, with private health insurance for the self-employed or above an income threshold.[23] As of 2016, 85% of the population is covered by the compulsory Statutory Health Insurance (SHI)[24] (Gesetzliche Krankenversicherung) (GKV), with the remainder covered by private insurance (Private Krankenversicherung (PKV). Germany's health care system was 77% government-funded and 23% privately funded as of 2004.[25] They may also opt for private insurance, which is generally more expensive, but whose price may vary based on the individual's health status.[26]

Insurer profitability in the individual market started to become much more widespread in 2017 and 2018. And although profitability is obviously the desired goal for insurance companies, they're not allowed to be too profitable. If their total administrative costs (including all overhead expenses plus profits) exceed 20 percent of the premiums they collect, they have to send rebate checks to their members. This is a provision in the ACA that ensures that health plans spend the majority of our premiums on medical costs, rather than administrative costs and profits.
Healthcare can contribute to a significant part of a country's economy. In 2011, the healthcare industry consumed an average of 9.3 percent of the GDP or US$ 3,322 (PPP-adjusted) per capita across the 34 members of OECD countries. The US (17.7%, or US$ PPP 8,508), the Netherlands (11.9%, 5,099), France (11.6%, 4,118), Germany (11.3%, 4,495), Canada (11.2%, 5669), and Switzerland (11%, 5,634) were the top spenders, however life expectancy in total population at birth was highest in Switzerland (82.8 years), Japan and Italy (82.7), Spain and Iceland (82.4), France (82.2) and Australia (82.0), while OECD's average exceeds 80 years for the first time ever in 2011: 80.1 years, a gain of 10 years since 1970. The US (78.7 years) ranges only on place 26 among the 34 OECD member countries, but has the highest costs by far. All OECD countries have achieved universal (or almost universal) health coverage, except the US and Mexico.[2][3] (see also international comparisons.)

Primary care refers to the work of health professionals who act as a first point of consultation for all patients within the health care system.[6][8] Such a professional would usually be a primary care physician, such as a general practitioner or family physician. Another professional would be a licensed independent practitioner such as a physiotherapist, or a non-physician primary care provider such as a physician assistant or nurse practitioner. Depending on the locality, health system organization the patient may see another health care professional first, such as a pharmacist or nurse. Depending on the nature of the health condition, patients may be referred for secondary or tertiary care.
HSA funds are considered to be “triple-tax advantaged.” This means any money put into the HSA account is contributed on a pre-tax basis, any earnings on the investments are not taxed, and any funds withdrawn for qualified medical expenses are not taxed. The HSA is an account owned by the employee, and the employee may choose to use HSA funds in the current plan year or roll the account balance forward to let it grow – even into retirement. Although the elimination of the individual mandate penalty and the expansion of short-term plans and association health plans are serving to drive premiums higher than they would otherwise have been in 2019, there are other factors, particularly when we look at rates on a state-by-state basis, that are causing rates to be lower than they would otherwise have been.
The Select PPO may be paired with a health care Flexible Spending Account (FSA) that can be used for qualified medical expenses during the plan year. During Open Enrollment, the employee chooses an amount to contribute in 2019 and then makes voluntary pre-tax contributions up to annual IRS limits. The funds in a health care Flexible Spending Account do not roll over from year to year; if funds are not used, they are forfeited. Vanderbilt does not contribute to health care FSA accounts.
Without digging into the nuances of Medicare Part D, I believe there are out of pocket maxes (similar to out of pocket maxes in commercial insurance plans). But you are right, these are not insignificant sums (~$5k – $10K). This is most definitely on my mind when it comes to retiring early and why I, not unlike PoF, am looking to “FatFIRE” to ensure I have plenty of cushion to cover these out of pocket maxes if I were to need to do so annually. This could come from my “retirement cushion”, cut back on vacay, or I may choose to do a little part-time work to help cover costs if something came up. Clinical EFT (Emotional Freedom Techniques) is an evidence-based method that combines acupressure with elements drawn from cognitive and exposure therapies. The approach has been validated in more than 100 clinical trials. Its efficacy for post-traumatic stress disorder (PTSD) has been investigated in a variety of demographic groups including war veterans, victims of sexual violence, the spouses of PTSD sufferers, motor accident survivors, prisoners, hospital patients, adolescents, and survivors of natural and human-caused disasters. Meta-analyses of EFT for anxiety, depression, and PTSD indicate treatment effects that exceed those of both psychopharmacology and conventional psychotherapy. Studies of EFT in the treatment of PTSD show that (a) time frames for successful treatment generally range from four to 10 sessions; (b) group therapy sessions are effective; (c) comorbid conditions such as anxiety and depression improve simultaneously; (d) the risk of adverse events is low; (e) treatment produces physiological as well as psychological improvements; (f) patient gains persist over time; (g) the approach is cost-effective; (h) biomarkers such as stress hormones and genes are regulated; and (i) the method can be adapted to online and telemedicine applications. This paper recommends guidelines for the use of EFT in treating PTSD derived from the literature and a detailed practitioner survey. It has been reviewed by the major institutions providing training or supporting research in the method. The guidelines recommend a stepped-care model, with five treatment sessions for subclinical PTSD, 10 sessions for PTSD, and escalation to intensive psychotherapy or psychopharmacology or both for nonresponsive patients and those with developmental trauma. State-run marketplaces / exchanges have more flexibility in terms of when they make new plans available for browsing. For example, Your Health Idaho, the state-run exchange in Idaho, debuted window-shopping for 2018 plans on October 2, 2018, nearly a month before the start of open enrollment, just as they did the year before. And Covered California is starting their open enrollment period (not just window shopping) two weeks early, on October 15. This will be the schedule they use in future years as well.
To clarify a small point, some high deductible (as high as $10,000 for family) plans that would be considered by many as “catastrophic plans” have been available AND Obamacare compliant. The compliance rules relate to the out of pocket maximum and other benefits rather than the deductible per se. furthermore, these plans are not necessarily cheap at all as many will tell you. I would not count on a huge break/savings once the Obamacare rules for Heath plans are no longer in play.

The UK's National Health Service (NHS) is a publicly funded healthcare system that provides coverage to everyone normally resident in the UK. It is not strictly an insurance system because (a) there are no premiums collected, (b) costs are not charged at the patient level and (c) costs are not pre-paid from a pool. However, it does achieve the main aim of insurance which is to spread financial risk arising from ill-health. The costs of running the NHS (est. £104 billion in 2007-8)[39] are met directly from general taxation. The NHS provides the majority of health care in the UK, including primary care, in-patient care, long-term health care, ophthalmology, and dentistry.
Lyme disease, caused by the spirochetal bacterium, Borrelia burgdorferi sensu lato (Bbsl), is typically transmitted by hard-bodied ticks (Acari: Ixodidae). Whenever this tick-borne zoonosis is mentioned in medical clinics and emergency rooms, it sparks a firestorm of controversy. Denial often sets in, and healthcare practitioners dismiss the fact that this pathogenic spirochetosis is present in their area. For distribution of Bbsl across Canada, we conducted a 4-year, tick–host study (2013–2016), and collected ticks from avian and mammalian hosts from Atlantic Canada to the West Coast. Overall, 1265 ticks representing 27 tick species belonging to four genera were collected. Of the 18 tick species tested, 15 species (83%) were positive for Bbsl and, of these infected ticks, 6 species bite humans. Overall, 13 of 18 tick species tested are human-biting ticks. Our data suggest that a 6-tick, enzootic maintenance cycle of Bbsl is present in southwestern B.C., and five of these tick species bite humans. Biogeographically, the groundhog tick, Ixodes cookei, has extended its home range from central and eastern Canada to southwestern British Columbia (B.C.). We posit that the Fox Sparrow, Passerella iliaca, is a reservoir-competent host for Bbsl. The Bay-breasted Warbler, Setophaga castanea, and the Tennessee Warbler, Vermivora peregrina, are new host records for the blacklegged tick, Ixodes scapularis. We provide the first report of a Bbsl-positive Amblyomma longirostre larva parasitizing a bird; this bird parasitism suggests that a Willow Flycatcher is a competent reservoir of Bbsl. Our findings show that Bbsl is present in all provinces, and that multiple tick species are implicated in the enzootic maintenance cycle of this pathogen. The federal government still isn’t funding cost-sharing reductions (CSR), but insurers and state regulators figured out a workaround last fall, and its use will be even more widespread for 2019. The details are explained here, but the short story is that the cost of CSR is being added to silver plan premiums in most states, and the CSR benefits themselves continue to be available in every state.

(US specific) Provided by an employer-sponsored self-funded ERISA plan. The company generally advertises that they have one of the big insurance companies. However, in an ERISA case, that insurance company "doesn't engage in the act of insurance", they just administer it. Therefore, ERISA plans are not subject to state laws. ERISA plans are governed by federal law under the jurisdiction of the US Department of Labor (USDOL). The specific benefits or coverage details are found in the Summary Plan Description (SPD). An appeal must go through the insurance company, then to the Employer's Plan Fiduciary. If still required, the Fiduciary's decision can be brought to the USDOL to review for ERISA compliance, and then file a lawsuit in federal court.
This year’s enrollment period offers good news to many Americans. After two years of carriers leaving markets and steep rate increases, states are seeing carriers re-enter exchanges for 2019 – and average rate increases are smaller than they were in 2017 and 2018.  And, although premium subsidies will be slightly decreased in 2019 (though not in all states), those eligible for cost-sharing reductions will continue to receive them.
