HSA funds are considered to be “triple-tax advantaged.” This means any money put into the HSA account is contributed on a pre-tax basis, any earnings on the investments are not taxed, and any funds withdrawn for qualified medical expenses are not taxed. The HSA is an account owned by the employee, and the employee may choose to use HSA funds in the current plan year or roll the account balance forward to let it grow – even into retirement. And if an employee leaves Vanderbilt, the HSA goes with them.
Then you may want to consider alternatives to ACA coverage like the HSA 5000, Premier Plans, Fixed-Benefit or AlieraCare. All of these alternatives give you nationwide coverage. There are very few ACA plans that will give you this nationwide coverage for anything other than a medical emergency. There are NONE in Arizona, South Dakota, or Texas (popular RVer domicile states). Florida Blue* remains a good ACA option for Florida residents that still allows members to use the national Blue Cross Blue Shield network when traveling outside of Florida.
Out-of-pocket maxima: Similar to coverage limits, except that in this case, the insured person's payment obligation ends when they reach the out-of-pocket maximum, and health insurance pays all further covered costs. Out-of-pocket maxima can be limited to a specific benefit category (such as prescription drugs) or can apply to all coverage provided during a specific benefit year.
One caution for any of you looking for cheap coverage – make sure the drug coverage is adequate. My best friend’s 77 year old husband was recently diagnosed with stage IV thyroid cancer (after full thyroidectomy and iodine treatment 11 years ago!) and his drug plan (he’s Medicare) won’t cover Tier 4 drugs at all, which is the only thing his doctor can offer. So he’s making his final arrangements because he’s going to die. I don’t know if this drug was going to just buy him some time or put him in remission or what, it’s new as of early 2018, but it’s heartbreaking to think he has no choices (other than suicide) because they cannot afford treatment. This drug costs, get this, $16,000 a month!!! That’s not a typo.
Nice discussion, Green Swan. One concern I have for young early retirees is that medical conditions accumulate as you and your family age. So, in the case of our host, PoF, in his early 40’s, he might be currently well served by a catastrophic plan. Ten years, fifteen years, from now, he and his wife might have some medical baggage that needs medication or maintenance, and the cat plan may not serve him well.

But for 2019, that uncertainty has not been a factor. The federal government will continue to not fund CSR, but states and insurers already know that. So the cost of CSR is almost universally built into the rates that insurers started filing in the spring of 2019 (in most cases, the cost is being added to silver plan rates, which is beneficial to the majority of consumers and can result in some highly discounted bronze and gold plans for people who qualify for premium subsidies)

Nearly one in three patients receiving NHS hospital treatment is privately insured and could have the cost paid for by their insurer. Some private schemes provide cash payments to patients who opt for NHS treatment, to deter use of private facilities. A report, by private health analysts Laing and Buisson, in November 2012, estimated that more than 250,000 operations were performed on patients with private medical insurance each year at a cost of £359 million. Private medical insurance does not normally cover emergency treatment but subsequent recovery could be paid for if the patient were moved into a private patient unit.[44]
The universal compulsory coverage provides for treatment in case of illness or accident and pregnancy. Minimum Essential Coverage should not be confused with Essential Health Benefits (EHB). EHB is a set of 10 categories of services health insurance plans must cover under the Affordable Care Act in order to be offered on the Marketplace. These include doctors’ services, inpatient and outpatient hospital care, prescription drug coverage, pregnancy and childbirth, mental health services, and more. Few factors bear as heavily on the well-being of any state’s citizens as their overall quality of health. The resulting programme is profession-based: all people working are required to pay a portion of their income to a not-for-profit health insurance fund, which mutualises the risk of illness, and which reimburses medical expenses at varying rates. If keeping your doctor and provider is critical to you, it’s important that you check now with the health insurance carriers in your area and your own doctors’ offices to get up-to-date network information. Carriers must update their provider directories at least monthly, and the directories must include information about which providers are accepting new patients, along with their specialty, location, and contact information. In addition, the directory must be easily available online without requiring the user to create an account or enter a policy number.
Probably not a surprise since we’re talking health insurance, but there really isn’t a great one-stop-shop. Insurance is regulated at the State level so insurers and plans will vary. I’d start with checking the major health insurers directly (Aetna, UnitedHealthcare, Anthem and Cigna) as they operate in many states. But there could be small insurers that offer CAT plans in your state as well so Google searching might be a good resort to find specific plans in your State (and buying them direct from the insurer). Wish I could be more helpful here.

Many consumers face unaffordable premiums – perhaps because they’re in the coverage gap or because their incomes make them ineligible for subsidies.
