That will continue to be the case in 2019, and the disproportionately large subsidies will be available in more places (for example, Vermont and North Dakota didn’t allow insurers to add the cost of CSR to premiums for 2018, but are allowing them to add the cost to silver plan rates for 2019, resulting in much larger premium subsidies. Colorado and Delaware required insurers to spread the cost of CSR across premiums for all plans in 2018, but are allowing the cost to be added only to silver plans for 2019, resulting in larger premium subsidies). So don’t pass up the opportunity to get a subsidy! Even if you’ve checked your eligibility before, make sure you do so again for 2019. As the poverty level rises each year, the income cap on subsidy eligibility also rises; it will be above $100,000 for a family of four in 2019.

If the subsidies eventually go away or if you are more of the “Fat FIRE” type (the high cost of living early retiree…) and don’t qualify for the subsidies, another option just got cheaper. With the repeal of the mandate, you can now buy what’s known as catastrophic health insurance (aka emergency health insurance or major medical insurance) without having to pay the mandate tax anymore.

You need to be relatively healthy to qualify for these plans. Any surgery in the past 6 months or scheduled in the next 12 months will likely disqualify you. If you are taking expensive medications at the time of application you will likely not qualify. ACA PPO plans are still hard to find and even if you do find one it does not necessarily make it a good choice for nationwide coverage, given the fact that their provider networks may be regional only (Like Avera Health in SD). So, once again we are offering alternative options for those that once something outside of the ACA offerings. The two biggest changes to our offerings in 2019 are 1) Short Term Medical can be written for up to 364 days (and renewed for up to 3 years) and, 2) Premier Plans (Elite Series) are back for self-employed individuals.
Recently (2009) the main representative body of British Medical physicians, the British Medical Association, adopted a policy statement expressing concerns about developments in the health insurance market in the UK. In its Annual Representative Meeting which had been agreed earlier by the Consultants Policy Group (i.e. Senior physicians) stating that the BMA was "extremely concerned that the policies of some private healthcare insurance companies are preventing or restricting patients exercising choice about (i) the consultants who treat them; (ii) the hospital at which they are treated; (iii) making top up payments to cover any gap between the funding provided by their insurance company and the cost of their chosen private treatment." It went in to "call on the BMA to publicise these concerns so that patients are fully informed when making choices about private healthcare insurance." The practice of insurance companies deciding which consultant a patient may see as opposed to GPs or patients is referred to as Open Referral. The NHS offers patients a choice of hospitals and consultants and does not charge for its services.
Can anyone address the elephant in the room: as medical therapeutics change and biologics are available and more appropriate for various conditions it is noteworthy to realize that these costs are often not covered by many government insurers and not eligible for foundations grants (as are sometimes offered in the form of copay cards, or copay assistance). I’m talking 20% out of pocket cost for a biologic can run 1500-2000 out of pocket after insurance. If you happen to get one of these rheumatologic or immunologic diseases, Medicare is NOT going to cut it. Are people folding in these possibilities into their projected costs in retirement. How does the FIRE community think about these things (I mean the medical FIRE community…I don’t think the non-medical FIRE community is even aware of these nuances unless they’re already dealing with a chronic or rare disease under treatment).
This has been very controversial. On one hand, people in that situation (i.e., having to pay full price for a health insurance policy in the individual market, which can easily cost 20+ percent of a person's income if they're just a little over the income limit for subsidy eligibility) are desperate for lower-cost alternatives. And if they're healthy, they may very well be willing to take a gamble and settle for a less robust plan that's easier to fit into their budget.
(US specific) Provided by an employer-sponsored self-funded ERISA plan. The company generally advertises that they have one of the big insurance companies. However, in an ERISA case, that insurance company "doesn't engage in the act of insurance", they just administer it. Therefore, ERISA plans are not subject to state laws. ERISA plans are governed by federal law under the jurisdiction of the US Department of Labor (USDOL). The specific benefits or coverage details are found in the Summary Plan Description (SPD). An appeal must go through the insurance company, then to the Employer's Plan Fiduciary. If still required, the Fiduciary's decision can be brought to the USDOL to review for ERISA compliance, and then file a lawsuit in federal court.
100 percent of qualified expenses (including pharmacy prescriptions) are paid by the employee until the employee deductible is met. Once the deductible is met, the employee will pay 20 percent co-insurance until the employee out-of-pocket maximum is reached. At that point, the Choice CDHP plan pays 100 percent of qualified expenses for the remainder of the plan year.

If you qualify for a subsidy and MONTHLY COST is the most important factor to you then a ACA plan is probably your best option since none of the other options can be used with a subsidy. If you do not qualify for a subsidy, however, you will probably find any of the other options offer a 30-70% lower cost option than an ACA plan.
Without digging into the nuances of Medicare Part D, I believe there are out of pocket maxes (similar to out of pocket maxes in commercial insurance plans). But you are right, these are not insignificant sums (~$5k – $10K). This is most definitely on my mind when it comes to retiring early and why I, not unlike PoF, am looking to “FatFIRE” to ensure I have plenty of cushion to cover these out of pocket maxes if I were to need to do so annually. This could come from my “retirement cushion”, cut back on vacay, or I may choose to do a little part-time work to help cover costs if something came up. Then you will want to consider either an ACA plan, HSA 5000, Premier Plans or the AlieraCare option since these options include FULL ACA-required preventive care with ZERO out of pocket costs to members. But, we advise against purchasing a plan solely based on this offering since the largest risk of loss with healthcare is not routine preventive care but rather extended hospitalization.
Short Term Medical plans are medically underwritten so they will be most suitable for healthy individuals without pre-existing conditions or expensive medications. Keep in mind that when you renew a STM plan you have to medically qualify each renewal term. So, if you have a medical situation occur while enrolled in a STM they can not drop your coverage but they can deny you the option to renew it at the end of your term.

During the 1920s, individual hospitals began offering services to individuals on a pre-paid basis, eventually leading to the development of Blue Cross organizations. The predecessors of today's Health Maintenance Organizations (HMOs) originated beginning in 1929, through the 1930s and on during World War II.
The insured person has full freedom of choice among the approximately 60 recognised healthcare providers competent to treat their condition (in their region) on the understanding that the costs are covered by the insurance up to the level of the official tariff. There is freedom of choice when selecting an insurance company to which one pays a premium, usually on a monthly basis. The insured person pays the insurance premium for the basic plan up to 8% of their personal income. If a premium is higher than this, the government gives the insured person a cash subsidy to pay for any additional premium.

So why are we hearing that average rates are decreasing? It turns out that average benchmark premiums (as opposed to overall average premiums) in states that use HealthCare.gov are decreasing slightly for 2019. The benchmark plan is defined as the second-lowest-cost silver plan in each area (it's also a term used to describe the basic set of benefits that must be covered in each area, but that's not the definition we're talking about here).
Outside of that time, you can qualify for Special Enrollment Period with “qualifying life events”. Some of these events include divorce, loss of employment, income change, new dependents, or moving to a new area. You will have to prove that you had a qualifying life event, and find health insurance within a certain window of time.
